School Nurse Self-Referral Form
If you are a young person and would like to talk and gain support from a School Nurse, you can complete this short self referral form. You can also ask a parent & carer  to make a referral for you, but you will need to give your permission for this. If you are under 12 years old then parent & carer will need to complete the referral with you.
